What is a Quality Improvement Plan?
A Quality Improvement Plan (QIP) is a formal, documented set of quality commitments aligned with system and provincial priorities that a health care organization makes to its patients/clients/residents, staff and community to improve quality through focused targets and actions.
The Vision of CHFHT is to empower all of our patients to enjoy healthier, happier and longer lives. Our mission statement and values strongly support the vision. Continued quality improvement is stated explicitly as one of our Values. Quality Improvement is well aligned with our Strategic Plan and is a priority for the Team. We see Quality Improvement as the combined and unceasing efforts of the Central Hastings Family Health Team to make the changes that will lead to better patient outcomes (health), better system performance (access/care) and better professional development (learning).
