What is a Family Health Team

Family Health Teams are healthcare organizations that include a team of family physicians, nurse practitioners, registered nurses, social workers, dietitians, and other professionals who work together to provide primary health care for their community. Primary health care refers to the patient’s first point of contact with a doctor or a healthcare team. Family Health Teams provide more service and a wide range of health options, especially for people who don’t have a doctor. Family Health Teams ensure that people receive the care they need in their communities. They focus on chronic disease management, disease prevention, disease cure, palliative care and health promotion, and work with other health care organizations, such as Public Health Units and Community Care Access Centres. Family Health Teams are patient centered and you will have access to a number of primary health care services from a variety of team members, including your primary care practitioner. For example:

  • Your primary care practitioner will continue to see you during regular office hours.
  • You will have access to other health care professionals within that Family Health Team, such as doctors, nurse practitioners, nurses, dietician, chiropodist and social worker.
  • You will have access to a Family Health Team doctor during extended evening and weekend hours for urgent problems.
  • If you see another primary care practitioner in your Family Health Team for treatment, your own primary care practitioner will receive documentation about the nature of your problem so that he or she will always be up-to date about your medical condition.
  • If you need assistance after-hours, you will be able to telephone the Family Health Team Telephone Health Advisory Service where a registered nurse will provide advice about your urgent health care concerns. With your permission, your primary care practitioner will receive a written summary of your call the next day, so that your primary care practitioner is informed of the nature of the problem and the advice you received.
