Do You Need a Family Health Care Provider?

The Central Hastings Family Health Team is pleased to announce they are participating in Health Care Connect. This is a program that has been created by the Ministry of Health and Long Term Care to assist you in linking with doctors or nurse practitioners who are accepting patients.
To become a patient of the Central Hastings Family Health Team you will need to join Health Care Connect (HCC). If you have no current health care provider, you will speak to someone who will ask you a short series of questions and sign you up for the program. Once you’ve joined Health Care Connect a nurse, called a Care Connector, will be assigned to help you find a health care provider in your area.
When speaking with Health Care Connect you may identify that you would like to become a patient with the Central Hastings Family Health Team, and even tell them which clinic, Madoc or Marmora, you would like to be directed to.
What if I already have a doctor and want to change over to CHFHT?
If you already have a physician you can still become a patient at CHFHT. You will need to de-roster with your current physician. This can be done by calling Service Ontario at 1 866 532 3161. Within 5-10 days you will receive a letter confirming that you are de-rostered with that doctor. Once you have the letter please call HCC at 1 800 445 1822. You will speak to someone who will ask you a short series of questions and sign you up for the program. Once you’ve joined Health Care Connect a nurse, called a Care Connector, will be assigned to help you find a health care provider in your area.

HCC will then forward a letter to the CHFHT and one of our administrative clerical staff will call you to book an appointment.
What if I need to see a doctor before my appointment?
In an emergency we recommend that you proceed to the nearest emergency department for care. If during this process you require urgent medical care, and CHFHT has received the paperwork indicating you are registered with Health Care Connect, one of our health care practitioners may be able to see you.
