Social Working – Part Time (.6 FTE)
The Central Hastings Family Health Team is currently recruiting a part time Social Worker to join their team.
The Central Hastings Family Health Team is a dynamic team of professionals. We are located in the heart of Hastings County with offices in Madoc and Marmora. The successful candidate will be working both sites. This is a .6 FTE permanent position.
We are seeking an energetic individual with the ability to work independently and within the Team framework. You will be working collaboratively with other members of the interdisciplinary team to provide healthcare to our patients.
You will be a key member to help restore, maintain and enhance emotional and social functioning with individuals and family within the Team. You will help assess, diagnose, treat and evaluate personal and family interpersonal and societal problems and to provide interventions and strategies to assist individuals, families, groups and communities to achieve optimum psycho social and social functioning.
You will have a Baccalaureate Degree in Social work. A Masters degree would be an asset. You will be registered with the Ontario College of Social Workers. A minimum of 5 years of experience in a healthcare setting an asset with an ability to provide counselling and basic psychotherapy.

RN – Program Coordinator / System Navigator
The Central Hastings Family Health Team is currently recruiting a 1 FTE Registered Nurse Program Coordinator / System Navigator to join their Team. You will be working at both the Madoc and Marmora clinics.
We are seeking an energetic individual with the ability to work well independently and within the Team framework. You will be working collaboratively with other members of the interdisciplinary team in the capacity of program management, healthcare navigation and information technology.
You will be involved in the development and management of health promotion and illness prevention strategies. You will provide support to the allied health professions and patients as a key member of the programs such as diabetes, COPD and health and chronic illness management. In addition, your medical administrative expertise will be utilized in the tracking and reporting of the team programs.
You will have two or more years of experience and current registration with the Ontario College of Nurses. You will have knowledge and experience in program development and evaluation. Your information technology and electronic medical record experience will provide a strong background to implement and co-ordinate IT hardware and software for all team members.

The position is open to patients from the Tri Area Medical Centre, the Marmora Medical Centre and the Gilmour Medical Clinic. Are you thinking “only if I had the time”?
